New Patients

We are now registering new patients, please recommend us to your friends and family.


Our practice area covers Harrogate town only. If you are unsure whether your address is within our boundary area, please check below.

To register, please call into the surgery to complete the registration forms, or the registration form is also available for download here, please make sure that you have signed it before submitting it to us.  Patients will be allocated a ‘usual GP’  but this does not mean that you can only see that particular doctor.  We can also email the forms to you if you prefer, contact us at to request this.

You will be required to complete a formal registration form for each member of the family, and as your medical records often take a while to come from your previous GP, you will be asked to complete a brief questionnaire. Please indicate if you are a carer.

There is also a form about Summary Care Records, please read this carefully and only complete this if you do NOT want your records to be shared with other healthcare professionals such as those at hospitals, should you require emergency treatment somewhere else in the country then they would not be able to see any allergies you may have or any current medications you are taking, so please bear this in mind when making your decision.

If you wish to register a new baby with us, then we will only need the registration form and not the questionnaire, however it is important to tell us the baby’s NHS number which can be found in the red book you are given shortly after the birth.

Temporary Residents

The practice is happy to see any friend or relative visiting a patient who is already registered with Park Parade and who requires urgent medical care which cannot wait until they return home. Similarly if you are a student home from University for holiday periods who has been registered here before,  then you can also be seen here for the duration of your break; in both instances please see Reception for the appropriate form to complete.
